Purpose of project (summary):

The Individual Consultant is expected to develop a comprehensive policy brief on the inclusion of refugees in the national health insurance program, ensuring alignment with government priorities. This process will build upon existing resources and analyses, including the 2024 Situation Analysis of Refugee Health Status: Challenges and Priority Interventions, as well as relevant documents on the inclusion of refugees in the national health insurance scheme and HIV services. The brief will then be disseminated to key stakeholders to inform and facilitate decision-making.

Following the development of the policy brief, the consultant will present the key recommendations to influential government stakeholders involved in decision-making or policy influence related to the inclusion program. These stakeholders may include, but are not limited to, the Ministry of Health, the National Social Security Council, and the Health Care and Social Security Agency. The presentation will be carried out through a well-structured workshop aimed at enhancing the inclusion of refugees in the access to the national health insurance scheme.
At the end of the project, the consultant will submit a comprehensive final report outlining prioritized recommendations for the inclusion strategy. The report will include a detailed list of stakeholders to be engaged, along with actionable points to ensure effective implementation.

General Background of Project or Assignment:

As of July 2025, over 11,972 refugees and asylum seekers are residing in Indonesia. The United Nations High Commissioner for Refugees (UNHCR) Indonesia advocates for ensuring refugees’ human rights and their access to national services in Indonesia, including access to healthcare and inclusion in the national health insurance program.

Refugees have access to primary healthcare facilities such as PUSKESMAS to be diagnosed and treated for basic health issues. Refugees are required to pay for the services at PUSKESMAS at the same rate as the Indonesian host community. Refugees are currently unable to access the national health insurance program and, as a result, access to secondary and tertiary healthcare for independent living refugees remains very limited. UNHCR, in partnership with our health partner, Yayasan Cita Wadah Swadaya (YCWS), is only able to financially cover the most urgent and emergency care, such as life-saving interventions, limited chronic illnesses, gender-based violence response, and psychosocial support.

Under the current arrangement, limited resources present a significant challenge in ensuring refugees have access to health services. As a result, many requests for care are unable to be granted as UNHCR must prioritize the most urgent cases.

While primary healthcare services are accessible to refugees, the limitations in financial resources present a significant challenge, particularly for those living independently. The lack of financial support for secondary and tertiary healthcare, such as hospitalizations, specialized treatments, and surgeries, leaves this population vulnerable and at risk. Without financial assistance from humanitarian organizations, many refugees are unable to afford these services, often leading to delayed or forgone medical care. This situation highlights the importance of establishing a more sustainable system for refugee health needs.

To overcome these barriers and ensure equitable access to healthcare for refugees, a strategic approach is needed to advocate for refugee inclusion in Indonesia’s universal health coverage (UHC) program - Jaminan Kesehatan Nasional (JKN). Integrating refugees into the JKN program would provide them with a more stable and comprehensive pathway to accessing healthcare services at all levels, from primary to tertiary care. This inclusion would be a step toward fulfilling the fundamental right to health for all and align with core international human rights principles. This action would demonstrate the principle of "leaving no one behind" and will strengthen Indonesia's position as a nation committed to the universal right to health, as recognized in documents like the Universal Declaration of Human Rights. To achieve this, a targeted advocacy effort is essential to inform and influence key government stakeholders about the benefits of this inclusion and to address any policy or administrative hurdles.

Concise and detailed description of activities, including measurable outputs and delivery dates, impact indicators (what qualitative and quantitative results would be achieved upon completion of individual contract), performance indicators (timelines, value of the service rendered in relation to the cost, timeliness) and work plan:

Duties and Responsibilities:

1. Conduct a comprehensive desk review and situational analysis to map the policy, legal, and operational landscape concerning refugee inclusion in Indonesia's healthcare system. This includes analyzing existing data, reports, and legal frameworks from government ministries, humanitarian agencies, and other relevant sources. Analyze existing information to identify key legal, policy, and implementation barriers, opportunities, and entry points for enabling refugee inclusion in the national health insurance program (JKN).

2. Based on the desk review and situational analysis, develop an evidence-based policy brief that presents a clear and compelling case for refugee inclusion in the JKN program. Include both the pathway to inclusion (changes in law/policy needed to enable access) and the advocacy arguments that would strengthen efforts for refugee inclusion in the system. Draft a stakeholder mapping document that identifies key government decision-makers and influencers (e.g., within the Ministry of Health, BPJS Kesehatan, and Dewan Jaminan Sosial Nasional).

3. Plan and facilitate a stakeholder workshop to present the policy brief and its recommendations to relevant government officials and other key actors.

4. Provide strategic advice and support to UNHCR and its partners on advocacy strategies and next steps for advancing the inclusion of refugees in the national health system.


Deliverables:

The Individual Consultant is expected to deliver the following:

1. Prepare a comprehensive policy brief that presents an evidence-based, and well-reasoned case for including refugees in the national health insurance program.

2. Prepare a stakeholder mapping document that identifies and assesses the interests, priorities, and potential influence of key government and civil society stakeholders.

3. Provide a summary report of the stakeholder workshop, including an attendance list, discussion notes, feedback from key participants, and a list of agreed-upon action points.

4. Prepare a comprehensive report that encompasses the findings from points 1 to 4 of duties and responsibilities, accompanied by an executive summary highlighting the key findings and recommendations. The report shall also capture comments and review from stakeholders through the workshop and the recommendations of inclusion strategy.

Qualification and Experience of the Individual Consultant (degree required, years of experience, field of expertise, language required):

Education: University level qualifications Bachelor’s (essential) or Master’s degree (desired) in a relevant area including but not limited to in medical, public health, health actuary, public policy or related field.

Desirable Skills:

• Demonstrates at least 5 years of experience in social protection, research assignment, and report writing. Previous work experience with relevant government institutions or in public health program and refugee context, will be a significant advantage.
• Demonstrated experience in undertaking projects similar in nature and level of complexity, knowledge and expertise in Indonesian health system and insurance scheme is desirable.
• Demonstrated experience assessing innovation programmes or pilots highly desirable.
• Experience working with UN agency or international organization, with a focus on human rights and/or refugee issues.
• Knowledge of the humanitarian landscape in Indonesia, and exposure to the work done with refugees and other forcibly displaced populations in the country is an advantage.
